[proxy] add end-to-end cost calculation matrix test

Runs the real parser → meter pipeline on the embedded pricing defaults
and asserts exact USD totals and cache splits for every metered wire
shape, including the field-reported Bedrock Sonnet 4.6 case ($0.022719
bare, $0.137199 with a 30,528-token cache write).
